A 55 year old female farmer by occupation came with chief complaints of shortness of breath , generalised weakness and itching

Chief complaints 

Shortness of breath, generalised weakness and itching 

History of present illness 

Patient was asymptomatic 1 year back and then she was diagnosed with anemia and shortness of breath grade 2, had 5 units of blood transfusions at oasis hospital Hyderabad. 

Five months back she had same symptoms and had 2 units of blood transfusions at Nalgonda government hospital. Hematuria

Subsidies while giving blood transfusions.

Past history: 

6 months back she was diagnosed with varicella zoster

Personal history

Diet: mixed

Appetite: low

Bowel and blabber : normal

Sleep: not adequate 

No addictions

Family history is not significant 

General examination 

The examination was conducted at a well lit and well ventilated room. The patient was conscious, cooperative and coherent. 

Moderately built 

Pallor- present 

Icterus - no

Cyanosis - no

Clubbing- no

Pedal edema - no
